Reporting Issues

If you experience or witness unacceptable behavior — or have any other concerns — please report it with this confidential incident report form. All reports will be handled with discretion.

This form sends an email to the advisor who is currently handling community issue reports – at present that advisor is Chelsea Stearns. Any information received will not be shared with other advisors or community members, unless you specifically request it be brought to someone else’s attention. Should you prefer a different point of contact, please see the “Reporting Issues” section of our Code of Conduct for more options.

When reporting a problem, please include:

  • Your contact information, if you require a response.
  • Names (real, nicknames, or pseudonyms) of any individuals involved, including any witnesses, if applicable. Should you prefer to remain anonymous when submitting, feel free to do so.
  • Your account of what occurred, and if you believe the incident is ongoing. If there is a publicly available record, please include a link.
  • Any additional information that may be helpful, including screenshots if applicable.

After filing a report, the advisor who received it will contact you personally, review the incident, follow up with any additional questions, and make a decision as to how to respond.

When handling any incident report, we will always respect confidentiality requests for the purposes of protecting victims of abuse.
